For students in need of additional support including needs relating to food insecurity, financial insecurity, housing insecurity or other forms of insecurity can be requested.
Dean Certification Form/Request for Disciplinary Records
For current or former students to request official verification of their disciplinary or conduct record for transfer, graduate, or professional school applications.
Disability Grievance Form
For students to report concerns or file a grievance regarding disability accommodations, accessibility barriers, or potential violations of Section 504 or the ADA.
Don't See Your Name on the Dean's List?
This may happen from time to time for different reasons such as incompletes, NE grades, etc. If you do not see your name on the Dean's List and believe you met the requirements, please complete this form and we will look into it.
Release of Information Consent Form (For DOSO Use Only)
This form allows the Dean of Students Office and other RPI offices to share limited student information with each other so they can better support you. You can withdraw this permission at any time by emailing the Dean of Students Office.
Request an Excused Absence
For students to notify the Dean of Students Office about absences due to illness, emergency, or other extenuating circumstances and to request faculty notification when appropriate.
Student Conduct Forms:
Interim Suspension Review Form
For students placed on an interim suspension who wish to request a review of that decision prior to a conduct hearing, in accordance with RPI’s Student Conduct Code procedures.
