For students to report concerns or file a grievance regarding disability accommodations, accessibility barriers, or potential violations of Section 504 or the ADA.
FERPA Release of Information Consent Form (DOSO Use Only)
Authorizes the Dean of Students Office to share specified educational records or information with designated third parties in accordance with FERPA regulations.
